Russian Population Health Coordinator/Community Health Worker (PHC/CHW)

Greater Lynn Senior Services (GLSS) is looking for a bi-lingual Russian speaking Community Health Worker (CHW).

The PHC/CHW will provide one-on-one supports to consumers in all general health issues under the guidance of a Nurse and Falls Prevention Specialist.  Supports can include personal coaching around daily routines including encouraging consumers to engage in exercise and other activities which improve balance.  These supports can be delivered in the home, at community kiosks or at other appropriate sites.  The PHC/CHW will also assist the falls prevention team in organizing group activities and delivering community education around critical falls prevention issues.

Job Description: Qualifications: Preferred experience in the health or social services field with an emphasis on community work, including conducting home visits with consumers.  Applicants must have knowledge of consumer engagement techniques and familiarity with general health and wellness strategies.  Strong interpersonal and coaching skills, ability to work in multi‐cultural environments and with consumers with diverse health, behavioral health, and both formal/informal support needs. Prospective applicants must demonstrate the capacity to effectively network with diverse community and regional agencies and resources. Candidates must have effective communication, interpersonal skills, and be proficient in databases such as Word, Excel and Outlook. Fluency in Russian a must. Reliable transportation required. Certification as a trained Community Health Worker preferred.  But if the right candidate is not trained as a CHW, we will certainly support his/her effort as part of employment.
